Picking a support for Medusa often means finding someone who can keep her alive until she comes online. Lich helps solve this problem directly. Frost Shield provides a layer of damage reduction that applies before her Mana Shield takes effect, making every point of her mana go much further when absorbing damage. This defensive boost is crucial, allowing a fragile early-game Medusa to survive ganks and secure the farm she needs to become a threat. The shield gives her a buffer that few other supports can provide, helping to smooth out her notoriously difficult early timings.

In teamfights, this pairing creates a difficult choice for the enemy team. Medusa wants to stand her ground, often using Stone Gaze to force enemies to stop moving. This creates a perfect setup for Lich to cast Chain Frost, which thrives on clumped and stationary targets. Opponents are caught between focusing the incredibly tanky Medusa, risking multiple bounces from the ultimate, or ignoring her and getting wiped by her damage output.
